FMEA (Failure Modes and Effects Analysis)Failure Modes & Effects Analysis (FMEA) is a formalised process of assessing the likelihood of failures and their severity. It should start in the design phase to ensure that potential failure modes and their associated causes have been considered and addressed. It seeks to quantify Severity, Occurrence and Detection, so as to be able to rank potential failure modes according to a Risk Priority Number (RPN). It is used extensively in the motor industry and is now spreading to other fields. It is best performed as a cross-functional team brainstorming session, starting early in the Design process, as conducting System and Design FMEA's may lead to design changes. It can also be used to identify Critical Characteristic. An example output is given below. Performing a Process FMEA can help to refine the manufacturing process.
