FMEA (Failure Mode and Effects Analysis)Originally used in the aerospace industry and then adopted and used extensively in the automotive industry, FMEA's can be a powerful tool for predicting and avoiding product problems in any industry, and more and more companies are incorporating FMEA's into their NPD process. An FMEA is a formalised process of assessing the likelihood of failures and their severity. It should start in the design phase to ensure that potential failure modes and their associated causes have been considered and addressed. It seeks to quantify Severity, Occurrence and Detection, so as to be able to rank potential failure modes according to a Risk Priority Number (RPN). It is used extensively in the motor industry and is now spreading to other fields. It is best performed as a cross-functional team brainstorming session, starting early in the design process, as conducting a System FMEA or a Design FMEA (DFMEA) may lead to design changes. It can also be used to identify Critical Characteristics. An example output is given below. Performing a Process FMEA (PFMEA) can help to refine the manufacturing process.
